Ang Maxicare PRIMA Consult ay isang primary-care plan na nagbibigay ng access sa consultations sa lahat ng Maxicare Primary Care Clinics.

Para sa mga naka-abot ng Alamat (Move It)/Platinum (Grab), narito ang mga kasama sa healthcare plan:
- Unlimited primary-care consultations (maliban sa psychiatry)
- Member rates sa piling services tulad ng mga laboratory tests, diagnostic exams, imaging at vaccines
Ito ay maaaring ma-avail sa lahat ng Maxicare Primary Care Clinics sa buong bansa.
ANO PA ANG PWEDENG IDAGDAG SA HEALTHCARE PLAN?
Kung may active Maxicare Healthcare Plan ka, maaari kang mag-avail ng add-ons:
- Annual Physical Exam
Kabilang ang CBC, Urinalysis, Fecalysis, Chest X-ray at Physical Exam - Aspire Care Life Insurance
Dagdag na insurance coverage para sa death, accidents o critical illness na valid ng isang taon - Dependent card para sa iyong pamilya
Hanggang 3 consultations (2 General, Family or Internal Medicine physicians, and 1 session with Specialist, (maliban sa psychiatry) at member rates sa mga piling serbisyo na valid ng isang taon
Ito ay maaaring ma-avail sa lahat ng Maxicare Primary Care Clinics sa buong bansa.
PAALALA: Hindi automatic na kasama ang add-ons at dependents sa iyong monthly plan. May hiwalay na bayad ang mga ito.

- Abutin ang Platinum (Grab) o Alamat (Move It) na tier target ng buwan
- Abangan ang tier announcement upang malaman ang mga kaagapay na magiging available sayo
- Ang mga Alamat o Platinum ay makakareceive ng email tungkol sa kanilang Maxicare member eCard at instructions kung sakaling nais magdagdag ng add-ons
- Ipakita ang iyong eCard at sundin ang clinic instructions sa iyong pagbisita
KAAGAPAY TIP: Siguraduhing updated at naa-access ang email na naka-link sa iyong Driver App. Huwag munang pumunta sa clinic para gamitin ang plan hangga’t hindi confirmed na active ang membership mo.

Pwede ko na bang gamitin ang Maxicare pagkakita ko ng tier announcement?
Hintayin sa iyong email ang confirmation na active na ang iyong membership at ang coverage dates nito.
Unlimited ba ang consultations?
May unlimited primary-care consultations habang active ang membership sa lahat ng Maxicare Primary Care Clinics sa buong bansa. Hindi kasama ang psychiatric consultations.
Libre ba ang laboratory tests at diagnostic exams?
Hindi. Ang piling laboratory, diagnostic, imaging at vaccine services ay maaaring available sa member rates, na babayaran mo nang hiwalay. I-confirm ang presyo sa clinic bago mag-avail.
Pwede ba sa kahit anong clinic?
Hindi. Sa Maxicare Primary Care Clinics lang available ang mga services na kasama sa healthcare plan. Kumpirmahin din ang availability ng kailangan mong serbisyo bago pumunta.
Kasama ba ang pamilya ko sa aking plan?
Hindi automatic. May optional dependent card na maaaring bilhin para sa pamilya, na may hanggang tatlong consultations sa Maxicare Primary Care Clinics at valid sa isang taon. Makikita ang instructions kung paano mag-avail nito sa email na iyong marereceive kasama ng iyong member eCard.
Kasama ba ang Annual Physical Exam (APE)?
Ang APE ay optional add-on na may hiwalay na bayad. Makikita ang instructions kung paano mag-avail nito sa email na iyong marereceive kasama ng iyong member eCard.
Ano ang AspireCare?
Isa itong optional life insurance add-on na hiwalay sa PRIMA Consult healthcare plan.Makikita ang instructions kung paano mag-avail nito sa email na iyong marereceive kasama ng iyong member eCard.
Sinasagot ba ng Maxicare ang lahat ng gastusin kapag naospital ako?
Hindi. Ang PRIMA Consult ay primary-care plan na may unlimited consultations sa Maxicare Primary Care Clinics. Hindi kasama sa coverage ang hospitalization.
Ano kung hindi ko ma-maintain ang Platinum o Alamat tier?
I-check ang end date ng kasalukuyang active access at ang susunod mong official tier notification. Ang eligibility sa susunod na cycle ay kailangang ma-validate ayon sa program rules.
Saan ko makikita ang aking membership eCard?
I-check muna ang email na naka-link sa Driver App, pati spam folder. Siguraduhing updated at naa-access mo ang iyong email. Huwag i-share ang account o membership details sa iba.
