Subic Bay Metropolitan Authority (SBMA) Grab Delivery-Partner Requirements

Bawat taon simula 2024, required na ang bawat delivery-partner na i-proseso ang requirements ng Subic Bay Metropolitan Authority (SBMA) upang patuloy na makabiyahe at makapag-deliver sa Subic Bay Freeport Zone nito. Ngayong 2025, siguraduhing makumpleto ito bago mag-October 31, 2025.

Upang mapadali ang pag-proseso nito, narito ang ating guide sa bawat requirement:

Sumailalim sa Medical Check-Up sa iyong piling clinic o hospital:

  1. CBC (Complete Blood Count)
  2. Hepatitis A test
  3. X-ray
  4. Urinalysis
  5. Stool Exam (Fecalysis)
  6. Physical Exam with Doctor’s Evaluation “Fit to work” 
  1. Address: Subic Bay Metropolitan Authority Building, 280 Dewey Ave, Olongapo City, 2200 Zambales

  2. Pagkatapos makuha ang iyong medical clearance, pwede na dumiretso sa Public Health and Safety Department (PHSD) Building.

  3. Dalhin ang xerox copy ng sumusunod na requirements:
    1. Motorcycle OR/CR
      1. Kung hindi sa’yo nakapangalan ang sasakyan:
        1. Letter of Authority o Deed of Sale
        2. Photocopy of Govt ID ng kung kanino naka pangalan ang CR with 3 specimen signature
    2. Driver’s License (front and back)

  4. Dumaan sa Motorcycle at Gear Bag inspection.

  5. Pagkatapos pumasa sa inspection, pwede nang bayaran ang sumusunod na fees:
    1. Sanitary Clearance: Php500
    2. Health ID: Php50

  6. Hintaying makatanggap ng SMS na available nang i-claim ang iyong Sanitary Clearance at Health ID.

  2. Pagkatapos makuha ang Sanitary Clearance at Health ID mula sa PHSD, pwede nang dumiretso sa Ecology Center para i-register ang iyong motorcycle.

  3. Dalhin ang xerox copy ng sumusunod na requirements:
    1. Motorcycle OR/CR
      1. Kung hindi sa’yo nakapangalan ang sasakyan:
        1. Letter of Authority o Deed of Sale
        2. Photocopy of Govt ID ng kung kanino naka pangalan ang CR with 3 specimen signature
    2. Driver’s License (front and back)
    3. Sanitary Clearance
    4. Health ID
  1. Address: Subic Bay Metropolitan Authority (SBMA) Office, Room 116, Bldg. 255, Barryman Road, Subic Bay Freeport Zone

  2. Hintayin ang SMS kung maaari nang dumiretso sa Labor Department para makuha ang iyong copy ng Certificate of Accreditation (COA).

  3. Sagutan ang Labor Form ng iyong kumpletong detalye.

  4. Hintayin ang “Control Number” mula sa Labor
    Department staff dahil hihingiin ito sa iyong SBMA ID Form.

  2. Dalhin ang xerox copy ng sumusunod na requirements:
    1. Motorcycle OR/CR
      1. Kung hindi sa’yo nakapangalan ang sasakyan:
        1. Letter of Authority o Deed of Sale
        2. Photocopy of Govt ID ng kung kanino naka pangalan ang CR with 3 specimen signature
    2. Driver’s License (front and back)

  3. Pumunta sa SBMA office para sagutan ang SBMA ID Form. PAALALA: Dito ilalagay ang iyong Labor Department “Control Number”

  4. Pagkatapos i-submit ang SBMA ID Form, bayaran ang sumusunod na fees:
    1. Kung first-time registration: Php100 (valid for 6 months)
    2. Kung for renewal: Php200 (valid for 1 year)

  5. Hintaying matanggap ang iyong SBMA ID sa mismong araw ng iyong registration o renewal.

  2. Pagkatapos makuha ang iyong SBMA ID, maaari nang dumiretso sa Motor Vehicle Registration Office (MVRO).

  3. Sagutan ang MVRO form at i-submit ang xerox copy ng sumusunod na requirements:
    1. Motorcycle OR/CR
      1. Kung hindi sa’yo nakapangalan ang sasakyan:
        1. Letter of Authority o Deed of Sale
        2. Photocopy of Govt ID ng kung kanino naka pangalan ang CR with 3 specimen signature
    2. Driver’s License (front and back)

  4. Hintaying makuha ang iyong Motorcycle Gatepass Sticker sa mismong araw.
  1. Paano kung nakumpleto ko na ang bawat clearance?
    ‘Wag na pansinin ang mensaheng ito. Maaaring gamitin na lamang ito bilang guide sa susunod na taon.

  2. Anong mangyayari kapag hindi ko nakumpleto ang requirements bago ang deadline?
    Hindi ka maaaring tumanggap ng bookings sa Subic Bay Freeport Zone. Simulan na ang pagproseso ng bawat clearance sa lalong madaling panahon upang maiwasan ito.
